The State of Connecticut requires past employers of employees who have applied for positions dealing directly with children in a school setting to complete their new 16-67 ACT form. You can read the ACT here.

Past employers can range, but the majority of them are other educational institutions. This means that when the hiring season begins (usually before the start of the new school year), these institutions are inundated with both sending and completing these requests.
